CALIBRE is seeking a Transition Counselor to support the Army Transition Assistance Program (TAP). The Transition Counselor serves as a trusted advisor and primary case manager for transitioning Soldiers and eligible clients, providing individualized counseling, group instruction, and comprehensive transition support throughout the military-to-civilian transition process. This position plays a critical role in helping clients successfully prepare for post-military employment, education, entrepreneurship, vocational training, and other career pathways.
The successful candidate will guide participants from initial counseling through Capstone validation, helping them develop and execute Individual Transition Plans (ITPs), complete Career Readiness Standards (CRS), and access available resources and support services. The Transition Counselor must be skilled in case management, adult learning, counseling, customer service, and stakeholder coordination while ensuring compliance with Army TAP policies and contractual requirements.
Summary
The Transition Counselor provides individualized counseling, group instruction, and lifecycle case management to transitioning Soldiers and eligible clients. This position serves as the primary case manager from enrollment through transition completion, helping participants achieve transition readiness, meet Career Readiness Standards, and access resources necessary for long-term success.
Key Responsibilities
- Conduct initial counseling sessions, assess transition readiness, interpret assessment results, and assign appropriate service tiers in accordance with Army TAP requirements.
- Assist clients in developing, updating, and executing Individual Transition Plans aligned with employment, education, entrepreneurship, vocational training, and personal goals.
- Deliver individual counseling, classroom instruction, workshops, briefings, and virtual services using Government-approved curriculum and instructional materials.
- Maintain continuous case management engagement and required follow-up throughout the transition process, from enrollment through Capstone completion.
- Guide clients in completing Career Readiness Standards and required documentation, including DD Form 2648 and associated records.
- Schedule and reschedule appointments, conduct client follow-up activities, identify participation barriers, and coordinate appropriate interventions.
- Document all services, assessments, milestones, referrals, warm handovers, and follow-on actions accurately and within established timelines.
- Coordinate referrals and warm handovers to the Department of Veterans Affairs, Department of Labor, educational institutions, employment resources, community organizations, and other support agencies.
- Support intake, registration, classroom logistics, attendance tracking, records management, participant surveys, and other administrative functions.
- Participate in quality assurance reviews, staff training, reporting activities, outreach initiatives, mobile support efforts, and surge coverage assignments as required.
